Samuel Leo Hazelwood, 79 of Horseshoe Bend, Arkansas, died Friday, January 13, 2006 at Fulton County Hospital in Salem, Arkansas. He was born in the Macy area near Monette, Arkansas. He retired as a Staff Sgt. from the U. S. Airforce.

He was preceded in death by his wife, Rene Hazelwood, his parents, J. A. & Lilly Bell Hazlewood, 3 brothers and 3 sisters.
