Chandler Lynn Sullins, of Jonesboro, was born at 4:21 p.m. and died at 4:31 p.m. Tuesday, November 6, 2012 at St. Bernards Medical Center.
He is preceded in death by his great grandmother Janice Culliper.
He leaves behind his mother Kayla Cook and father Zachary Sullins; a sister Karissa Cook; a brother Brysun Sullins all of Jonesboro.
He also leaves his grandparents Pamela and William Cook of Jonesboro and Vernon Sullins of Jonesboro.
A memorial service will be held Saturday, November 10, at 2:00 pm at Children’s Bible Church on Woodrow.
